DEPARTMENTAL SUMMARY: St. Charles Anticoagulation clinic is the largest provider of anticoagulation services in Central Oregon. We serve patients living in Central and Eastern Oregon. Our staff of RN's and LPN's monitor INRs in clinic via point of care testing and manage dosing according to clinic protocol and under the direction of APP. We have a growing population of patients who home monitor their INRs and dosing is managed over the phone.
POSITION OVERVIEW: Responsible for performing clinical assessments and treatment plans for patients within the Anticoagulation clinic. Working under the direction of the Anticoagulation medical director and within the current Anticoagulation protocol, the APP will complete new patient intakes and yearly evaluations for anticoagulation therapy while working with ordering providers to recommend the best therapy options for patients needing anticoagulation. Provide patient education for new patients in the anticoagulation clinic and patients having changes in treatment. Responsible to evaluate, recommend and order perioperative anticoagulation plan following current perioperative guidelines. Will be responsible to work with the nurses in the anticoagulation clinic to direct care for patient with INR outside of range. Responsible for review and cosign of all anticoagulation encounters conducted in the Anticoagulation Clinic and over the phone. Will work in conjunction with the Anticoagulation Medical Director and the Manager of Clinic Operations to develop additional services, education within the clinical team and community awareness of service offered within the anticoagulation clinic.
Work schedule: The Anticoagulation Advance Practice Provider will work a minimum of 40 hours per week
Call Schedule: APP will participate in clinic call from satellite clinics during normal working hours. No after- hours call required.
APP Practice Location: APP will render the Professional Services at such SCHS Facilities as may be directed by SCMG, and at such other facilities and locations as may be established by SCHS or SCMG from time to time. It is anticipated that the primary work locations will be as follows:

ESSENTIAL FUNCTIONS AND DUTIES:
Administrative Duties:
- Clinical Leader for the Anticoagulation Clinic Program Services. As the Clinical Leader for the Anticoagulation Clinic Program, Nurse Practitioner will perform the following duties:
- The APP lead will be a leader among the APPs.
- Address human resource issues with APP Staff.
- Assist in communication and quality improvement.
- Help develop performance improvement plans for APPs, RNs and LPNs as needed.
- Help with orienting new APPs, RNs, LPNs to system and department policies.
- Serve on the SCMG - wide Clinical Caregiver Professional Standards Task Force and implementation teams.
- Plan, design, develop, implement, and evaluate population-based anticoagulation programs, including anticoagulation stewardship.
- Design, develop, implement hybrid (centralized management, decentralized patient services) anticoagulation services in collaboration with Primary Care and Cardiovascular Services clinical divisions.
- Serve as a member of SCMG nursing / clinical caregiver professional development task forces or committees in support of nursing and medical assistant professionals.
- Track and complete root cause analysis for adverse events
Clinical Duties:
- Performs patient examinations, obtains, and records pertinent medical history, subjective and objective findings. Orders and interprets appropriate laboratory, imaging, and other diagnostic procedures. Interprets and integrates data to determine diagnosis and therapeutic plan.
- Administers and prescribes medications in compliance with state law and practice guidelines.
- Orders and performs appropriate medical and surgical procedures in accordance with State law and practice guidelines.
- Instructs patient and family regarding medications and treatments. Educates patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.
- Collaborates with physicians and other caregivers to manage the acute and long-term health needs of patients.
- Provides monitoring and supports continuity of care between clinic visits.
- Maintains and reviews patient records, charts, and other pertinent information, documenting clearly and concisely.
- Charts tests, examination results, and other appropriate clinical data.
- Triages patient telephone calls, providing consultation as needed.
- Communicates effectively with colleagues, patients, physicians, and administration.
- Maintains quality of clinical practice by selecting appropriate avenues for professional growth.
- Medical performance issues will be directed to the medical director of the service. All other performance issues will be directed to the practice or department director.
- May perform additional duties of similar complexity within the organization, as required or assigned.

Complies with all applicable laws, regulations, policies, and procedures, supporting the health system's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all health system's investigations and proceedings.
- Keep all licenses and appropriate certifications current and unrestricted to maintain privileges at a St. Charles Health System hospital sufficient to permit performance of job duties.
- Increases understanding and collaboration throughout the medical community.
- Maintain compliance of hospital's Medical Staff Bylaws, Rules and Regulations, as required by SCHS Board of Directors.
EDUCATION
Required:
- Successful completion of an approved Nurse Practitioner course of study and certification as appropriate to the incumbent's license.
- Complete annual fire/safety, etc. education in-services as required.
- Attend mandatory educational offerings as described in SCHS policies/procedures and/or as required by other regulatory bodies.
- Complete annual CME as required to maintain license and appropriate Board certification.
- Complete in a timely manner assigned Computer Based Learning modules.
Preferred:
LICENSURE/CERTIFICATION/REGISTRATION
Required:
- Unrestricted Nurse Practitioner licensure.
- Nurse Practitioner requires AANC, NCC or AANP, AMCB certification.
- Unrestricted Federal DEA number
- Unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs.
- Valid driver's license and the ability to meet SCHS driving requirements.
- Current BLS certification required.
- If provider will practice sedation, moderate or deep, ACLS certification is also required.
- Additional certifications may be required based on privileges granted to provider by Medical Staff Services at SCHS Inpatient locations.
Preferred:
- Oregon MSN licensure preferred.
- Federal DEA number registered in Oregon.
EXPERIENCE
Required:
- 0-2 years' experience as an Advanced Practice Provider.
- Experience using EMR (Electronic Medical Records) software
- Advanced written and verbal communication skills to effectively interact with a diverse population and professionally represent St. Charles Health System
- Experience working in a team environment and proven ability to collaborate, manage facilitation and consensus building among health care professionals and agencies to achieve expected results
Preferred:
- 2+ years' experience as an Advanced Practice Provider.
- Epic EMR software experience
PERSONAL PROTECTIVE EQUIPMENT
- Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
